Customization Questionaire

Let's get this ball rolling!  By answering the questions below, we should be able to get a better idea of what you want and be able to provide you with a quote!  Because each piece is so unique it may increase the cost of the item depending on the media you would want to be the focal point.  For example, if you would like a bath board that you want made with a few of her favorite things like rose petals then there may be an extra charge to go out and purchase the rose petals.  Please be as specific as possible.  Once we receive your questionaire responses we will be in contact to put the finalized touches on the item.
